玩家掌控的是一位没有名字的太空陆战队员，“地球上最顽强、为战斗受高度训练”的人，由于拒绝服从上级命令杀害无武装的平民并攻击长官，因而获罪被放逐到火星。他被迫替军事工业企业集团UAC（Union Aerospace Corporation，联合宇宙航空公司）工作，公司正在火卫一和火卫二之间执行瞬间移动（teleport）的秘密实验。一次实验时因为突然出错，来自地狱的生物经传送门蜂拥而出，防卫单位猝不及防，使恶魔们迅速占领整个基地，所有的人员不是被杀就是成为丧尸，同时火卫二完全消失。于是UAC派遣一支队伍到火卫一调查此事，但不久队员之间的无线电便转为静默，显示各人已被杀害，剩下的唯一一个人类所能做的便是设法逃出生天。
