Romance of the Three Kingdoms V ist einer der bekanntesten klassischen Teile in Koeis historischer Strategieserie und steht für viele Spieler für den Höhepunkt des frühen DOS-Ära-Stils. Während des Zusammenbruchs der Späteren Han und des Aufstiegs der Drei Reiche angesiedelt, versetzt das Spiel den Spieler in die Rolle eines Herrschers, der Städte regieren, Offiziere ernennen, Diplomatie betreiben, Krieg führen und letztlich China einigen muss. Besonders unverwechselbar war das "Reputation"-System, bei dem die Unterstützung der Bevölkerung direkt beeinflusste, wie viele Aktionen ein Herrscher durchführen konnte, wodurch Innenpolitik und öffentliche Stimmung deutlich mehr strategisches Gewicht erhielten. Kämpfe wurden durch zwölf Formationen und ein breiteres Spektrum an Offiziers-Kampffähigkeiten ebenfalls vielschichtiger, während das zusätzliche Szenario der Gelben-Turban-Rebellion die spielbare Zeitlinie erweiterte. Spätere Windows-, Saturn- und PlayStation-Versionen blieben sogar dadurch in Erinnerung, dass sie Filmmaterial aus der TV-Serie Romance of the Three Kingdoms von 1994 in Eröffnungs- und Ereignisszenen verwendeten, was dem Spiel eine besonders dramatische historische Atmosphäre gab.
