Romance of the Three Kingdoms V é uma das entradas clássicas mais reconhecíveis da série de estratégia histórica da Koei e, para muitos jogadores, representa o auge do estilo inicial da era DOS. Ambientado durante o colapso dos Han Posteriores e a ascensão dos Três Reinos, o jogo coloca o jogador no papel de um governante que deve administrar cidades, nomear oficiais, conduzir diplomacia, travar guerras e, por fim, unificar a China. O que tornou esta edição especialmente distinta foi seu sistema de “Reputação”, no qual o apoio popular afetava diretamente quantas ações um governante podia realizar, dando muito mais peso estratégico à política interna e ao sentimento público. As batalhas também se tornaram mais elaboradas com doze formações e uma gama maior de habilidades de combate de oficiais, enquanto a adição do cenário da Rebelião dos Turbantes Amarelos ampliou a linha do tempo jogável. Versões posteriores para Windows, Saturn e PlayStation até se tornaram memoráveis por usar imagens da série de TV Romance of the Three Kingdoms de 1994 em suas cenas de abertura e eventos, dando ao jogo uma atmosfera histórica especialmente dramática.
